cruisers 455 Maxwell anchor

I recently anchored in a bay. When I went to raise the anchor, it would get right to the top then release a lot of my chain. after 3 attempt's I finally was able to raise it. Does anyone know what the problem might be?

It sounds like the windlass clutch needs to be adjusted tighter.
