Looks like a reasonably system. Could use a vented loop for safety on the water inlet,between bowl & flushing pump.
Is your tank mounted above your toilet,or is it above when she climbs out or rolls down?
I'm lost,except for plugged vent,leaky duckbill or unknown pressure source-all mentioned by other posters.

Tank is mounted below toilet, maybe 6-8 ft of very old hose. I’m due for installing a vented loop here soon, just need more info before I go about that. I’m still not exactly sure where my waterline is in reference to my toilet and tank, I know that’s important in the placement of a vented loop. Anyway, thanks for the suggestions.

If the tank is below the toilet and the toilet only can flush into the tank you don't need a vented loop between toilet and tank.
When a toilet can flush overboard, and the toilet is near the water line then you do need a vented loop on the discharge.
Be thankful that your toilet can flush DOWN into a tank, it makes servicing a toilet SO much easier.
With toilet close to water line, you want a vented loop on the INLET water, as was mentioned in an above post, such vented loop between inlet pump and bowl.

Could be scaled up to the point where it is nearly blocked. You don't need a vented loop if the tank is below the toilet outlet.
