If this is on a dinghy, you want the cleat to
release downward, so if you are falling
overboard with the sheet in your hand,
or even tangled around you, you want it
to release. That won't happen if it releases
up.

You can turn it over also. Most racing boats are rigged such that you pull up to get the rope out of the cleat. i.e. a quick kick helps if you have a lot of load on the mainsheet and if necessary you can stand up to get jerk it out of the cleat. An upside down cleat on the deck may not be safe in a blow or critical situation as it will be difficult to release.

Just align the cleat so when you pull on more sheet the rope stays in the jaws of the cleat but when you pull a bit higher the rope slides out of the cleat.

Yes those shackles take a huge load. I too straightened one of those twisted shackles on the traveller rig.
I removed the shackles and used several turns of dyneema. This enabled me to raise the block to a safer position enabling crew to 'pull in and down'. I can also manage the main sheet from behind the helm safely. It's been like this for six years now - is higher, safer, quieter.
