Yes we are from Montreal, but we have in Puerto Aventuras FRENCH friends and they FEAR to be directed to French Islands rather than in their home town.

This is not the case with Canada that does not have "land in the south"... hopefully Canada did not annex Turks & Caicos... YES this was a serious projects some years ago ! probably to have his own fiscal paradise.

There used to be traveler's evac insurance available quite reasonable. Covering much of the world. Maybe google that.

Also check Global Rescue. They are very high end but their reputation is stellar. Although they provide services to National Geographic expeditions and many high end corporate clients, their consumer products are reasonably priced given what you get.

You really need to read the entire contract on all these evacuation or international insurance policies. Many just don't cover the most likely needs of a cruiser.
That said, the DAN policy is pretty good and they tend to interpret it in the customers favor.

Given the costs of health care in the USA I have been researching evacuation insurance to get me back to Mexico if I need hospitalization. Haven't found anything as yet.

In my limited understanding of what I have seen, most of these policies will evacuate you to the closest medical facility where you can receive appropriate immediate care.
Repatriation, bringing you back to your home city in the US, is secondary and separate from the initial medevac.
You'd have to check individual policies and specify that you want repatriation, not just immediate and appropriate care.
Insurers cleverly are reluctant to allow anyone to READ a policy BEFORE they buy it, instead they like to say buy it and you have 30 days in which to cancel and file for a refund. Yeah, right.
