General Info: Declaration of Conformity for Lagoon 410 needed

Hi,
I have a 2005 Lagoon 410 S2 which I'm trying to get into Survey in Australia.
I have page one of the Declaration of Conformity, but didn't get page 2.
I've contacted the builder CNB and they sent me page 2 but it was like a spreadsheet with all the Standards categories but it wasn't filled out. I asked them about it and they replied that it wasn't until 2006 that the standards were filled in.
Would anyone have a Declaration of Conformity for a 2006+ Lagoon 410 S2 (or a similar Lagoon will help) that has page 2 filled out. I can then send it to CNB and they will confirm that these were the Standards that my Lagoon was built to.
Then I can submit everything to my surveyor and I can get into survey.
