transfer sailing boat from north/west Europe to Croatia

Hello!

I would much appreciate if someone would guide me to the best solution of how to transfer (by road) 39ft sailing boat from the North/West (Holland, Germany or France...) to Croatia?

I am owner of a Van de Stadt Zeeton 24 since 2012 (wooden "home made"), and the time has come to go more serious with up to 70k eur.
