Pacific Seacraft builder's certificate

Can anyone tell me precisely what information is listed on a Pacific Seacraft builder's certificate? This is the document issued by the builder certifying where the boat was built, when it was built, its dimensions and so forth. I assume it includes length, width and height (not the same as draft), but what else is on there?

If anyone could let me see his/her certificate (with private information removed, of course), I would be EXTREMELY grateful. If it's for a PS37 of fairly recent vintage, I might have to come thank you in person.

I know that this is an unusual request, but I need this information ASAP, I have not been able to find it elsewhere (although I found a standard 'homeland security' document, but that's not what I need), and the PS person who does this sort of thing sadly died very recently (no kidding).

And why do you need this cert. If your trying to document the boat in the US, it's not necessary. If something else then let us know. There maybe a solution.
