Might help to suggest a repair method if we can see what we are looking at.
Not good that rebuilders didnt replace as it must have been clearly worn when they did it but that's water under the bridge now.

I repaired this same problem on a 3QM reduction gear and the selector yoke and collar was worn to a 3mm gap. The problem was no reverse and the fix was a second hand yoke and collarClick image for larger version

ID:	199319 this is a poor photo but the wear ledge is visible. The yoke and slider are available as hurth parts but I didn't realize that until I got it apart and recognised all the familiar bits. I still don't quite understand how the wear occurred, initially thinking it was bad adjustment but having read the OP I suspect that something else might have caused light but constant contact between the two parts..... in his case it caused un commanded selection of drive.
