Here's one to add to the "required" list: Ecuador (if visiting the Galapagos at least).

I drew the short straw for and upcoming Pacific crossing and am handling the Galapagos clearance. One of the listed requirements is liability insurance (presumably to cover any environmental clean up liability incured by the yacht).
