Re-entering the USA After 7 Years Away

We need to know where on line we fill out the form to re enter the USA. We got BR numbers early this year and have promptly lost the info on where we go to fill out and email the. Form so we just have to make a phone call when we get to Florida in Dec after our Atlantic crossing. We plan to sail right to Fla and not stop in the Caribbean. Can anyone help. I thought I'd read info here but a search did not bring it up. Many calls to immigration did not connect us to a real person...Skype is Crap in Spain.
